What is a Notice of Filing Petition for Modification of Legal Decision Making?

This is a simply 1 page form that must ALWAYS be filed when your Petition to Modify contains a request to modify legal decision-making.  If you do not provide this form at the time of the filing of your Petition, the Clerk will not file your Petition until you provide the form.  The Notice explains to the other party that they have 20 days to file a Response from the date they were served with the Petition and this notice and any other documents filed with your Petition.  If no Response is filed, the requesting party can ask the Court to set a hearing so that they can decide if the modification of legal decision-making should be granted.

This is a standard form that contains the same information in every case where one is filed.
